Phlebotomy

1. High School Graduate or received a high school equivalency certificate.
2. Successfully complete background check and drug screen.
3. Must be 18 years or older.

Health and Immunization Requirements

1. Physical Examination Form-Completed by a Healthcare Provider
2. The immunizations below
   • Tetanus or Tdap (Within 10 years)
   • Influenza
   • Measles, Mumps, Rubella (2 Immunizations or Positive titers)
   • Varicella (2 immunizations or positive titer)
   • Hepatitis B (Series of 3 immunizations or positive titer or a signed letter of declination)
   • COVID vaccine
3. Results of two TB skin tests within the past 12 months of attending clinical rotations, or negative chest X-ray results within the past five years for a history of positive TB skin test.
4. Criminal background checks/drug screening exams per clinical agency requirements (NOT required for HCC admission must use our department form.
5. Must have a paid subscription to Rotation Manager for clinical rotations.
6. Must have liability insurance purchased through the College.
